Hepatitis C infection is a global health problem affecting 3% of the world’s population. 1 In addition to causing liver diseases like cirrhosis and carcinoma, hepatitis C infection has been associated with wide variety of extrahepatic manifestations with multitude of disease processes affecting the small vessels,skin, kidneys,salivary glands, eyes,thyroid gland and immunologic system. The majority of these conditions are thought to be immune-mediated. Other HCV-associated entities include porphyria cutanea tarda, lichen planus, necrolytic acral erythema, membranous glomerulonephritis, diabetic nephropathy, B-cell non-Hodgkin lymphomas, insulin resistance, sialadenitis, sicca syndrome and autoimmune thyroiditis. However, multiple extrahepatic manifestation of hepatitis C in a single patient have never been reported till now. Here we are reporting one such case of Hepatitis C with multiple rare extrahepatic manifestations.

Background and Aims: Genetic susceptibility to the development of chronic obstructive pulmonary disease might depend on variation in the activities of enzymes that detoxify cigarette smoke products. We studied the relationship of GSTP1, GSTM1, and GSTT1 gene polymorphisms with COPD risk in a case-control study of Indian patients and controls. Material and Methods: A total of 186 patients with COPD and 160 healthy controls were included in the study. The frequencies of GSTP1, GSTM1 alleles were determined by using conventional multiplex PCR and GSTP1 by polymerase chain reaction and restriction fragment length polymorphisms technique. Results: A significant case-control difference was observed for the presence of null GSTM1, (61.8% vs 55.0%, P=0.04). No difference was observed in the frequency of GSTT1Null genotype and COPD susceptibility (54.8% vs 50.6% OR: 1.26; CI:0.87-1.84; P value=0.82). For GSTP1 polymorphism, we found that subjects homozygous variants Val/Val were at increased risk of developing COPD (OR: 2.58; CI: 1.2-4.8) as compared to heterozygote variants Ile/Val (OR: 1.28 CI: 0.7-2.14). Also, the mutant allele frequency (Val) was significantly higher in patients as compared to controls and the difference was found to be statistically significant. (OR: 1.8 CI: 1.4-4.2; P Value=0.001). Conclusion: We propose that subjects with GSTM1 null allele and GST P1 homozygous isoleucine genotypes are at higher risk of COPD and are significant indicators of susceptibility to chronic obstructive pulmonary disease in Indian population.

Neuroendocrine tumors of the larynx are the most common non-epidermoid tumors of the larynx and comprise less than 1% of the laryngeal tumors. Most of the symptoms and presentation mimic any usual laryngeal malignancy making the diagnosis difficult. Here, we report a case of laryngeal neuroendocrine carcinoma that was managed with total laryngectomy.

We present a case report of a patient with very aggressive course of anaplastic largecell lymphoma. The patient had nonspecific complaints of easy fatigability and progressive breathlessness and had generalized lymphadenopathy. Initial investigations revealed pancytopenia. Bone marrow examination revealed presence of atypical cells. Liver biopsy showed portal tracts infiltrated by atypical lymphoid cells. Fine-needle aspiration of the lymph node finally confirmed anaplastic large-cell lymphoma. Patient succumbed to the illness.

A 36-year-old nonalcoholic gentleman presented with history of moderate-amount hematemesis. He received multiple blood transfusions. upper gastrointestinal endoscopy was done which showed no evidence of varices, but a tiny bleeding ulcer was seen in lower end of esophagus;sclerosing agent was injected at the base of ulcer. All the blood investigations were normal. A diagnosis of Dieulafoy’s lesion was made. After two months, he developed hemarthrosis after sustaining injury in right knee. Again investigations were done and coagulation profile revealed raised activated partial thromboplastin time and normal prothrombin time. Factor VIII and IX assay were sent in view of raised aPTT. Factor VIII and IX was 120% and 4% of normal respectively. Von Willebrand factor levels were within normal limits. Revised diagnosis of moderate hemophilia B was made

Celiac disease is an autoimmune disorder of the small in testines.It may be associated with chromosomal syndromes like Downs, Turner and Williams syndrome.However, its association with a balanced translocation has never been described to our knowledge. We report a case of a female child who during evaluation for short stature was found to have celiac disease. Genetic testing later revealed a balanced translocation 8; 17. She responded satisfactorily to the dietary therapy of celiac disease
